On a U-100 insulin syringe, 0.25 mg of semaglutide is 25 units at 1 mg/mL, 12.5 units at 2 mg/mL, 10 units at 2.5 mg/mL, 6.3 units at 4 mg/mL, 5 units at 5 mg/mL, 2.5 units at 10 mg/mL.

At 2.5 mg/mL, the strength most often seen on compounded labels, it is 0.1 mL, which is 10 units. Use the calculator with the strength printed on your own vial.

Real vials can hold slightly more or less than the label says, and a little is lost in the needle.

Same dose, different units

At 2.5 mg/mL, 0.25 mg is 10 units. Drawing those same 10 units from a 5 mg/mL vial gives 0.5 mg, not 0.25 mg. Always check the mg/mL on the vial you are using.

In July 2024 the FDA warned about dosing errors with compounded semaglutide, including mix-ups between mg, mL and units (FDA alert). The same math applies to any vial.

Questions

How many mL is 0.25 mg of semaglutide?

It depends on the vial: 0.25 mL at 1 mg/mL, 0.125 mL at 2 mg/mL, 0.1 mL at 2.5 mg/mL, 0.063 mL at 4 mg/mL, 0.05 mL at 5 mg/mL, 0.025 mL at 10 mg/mL.

Is 0.25 mg of semaglutide the same number of units on every vial?

No. The units depend on the strength printed on the vial: 25 units at 1 mg/mL, 12.5 units at 2 mg/mL, 10 units at 2.5 mg/mL, 6.3 units at 4 mg/mL, 5 units at 5 mg/mL, 2.5 units at 10 mg/mL.
