On a U-100 insulin syringe, 10 mg of tirzepatide is 200 units (more than a 1 mL syringe holds) at 5 mg/mL, 100 units at 10 mg/mL, 66.7 units at 15 mg/mL, 58.8 units at 17 mg/mL, 50 units at 20 mg/mL, 40 units at 25 mg/mL, 33.3 units at 30 mg/mL.

At 10 mg/mL, the strength most often seen on compounded labels, it is 1 mL, which is 100 units. Use the calculator with the strength printed on your own vial.

Real vials can hold slightly more or less than the label says, and a little is lost in the needle.

Same dose, different units

At 10 mg/mL, 10 mg is 100 units. Drawing those same 100 units from a 20 mg/mL vial gives 20 mg, not 10 mg. Always check the mg/mL on the vial you are using.

In July 2024 the FDA warned about dosing errors with compounded semaglutide, including mix-ups between mg, mL and units (FDA alert). The same math applies to any vial.

Questions

How many mL is 10 mg of tirzepatide?

It depends on the vial: 2 mL at 5 mg/mL, 1 mL at 10 mg/mL, 0.667 mL at 15 mg/mL, 0.588 mL at 17 mg/mL, 0.5 mL at 20 mg/mL, 0.4 mL at 25 mg/mL, 0.333 mL at 30 mg/mL.

Is 10 mg of tirzepatide the same number of units on every vial?

No. The units depend on the strength printed on the vial: 200 units (more than a 1 mL syringe holds) at 5 mg/mL, 100 units at 10 mg/mL, 66.7 units at 15 mg/mL, 58.8 units at 17 mg/mL, 50 units at 20 mg/mL, 40 units at 25 mg/mL, 33.3 units at 30 mg/mL.
