On a U-100 insulin syringe, 12.5 mg of tirzepatide is 250 units (more than a 1 mL syringe holds) at 5 mg/mL, 125 units (more than a 1 mL syringe holds) at 10 mg/mL, 83.3 units at 15 mg/mL, 73.5 units at 17 mg/mL, 62.5 units at 20 mg/mL, 50 units at 25 mg/mL, 41.7 units at 30 mg/mL.

At 10 mg/mL, the strength most often seen on compounded labels, it is 1.25 mL, which is 125 units (more than a 1 mL syringe holds). Use the calculator with the strength printed on your own vial.

Real vials can hold slightly more or less than the label says, and a little is lost in the needle.

Same dose, different units

At 15 mg/mL, 12.5 mg is 83.3 units. Drawing those same 83.3 units from a 30 mg/mL vial gives 25 mg, not 12.5 mg. Always check the mg/mL on the vial you are using.

In July 2024 the FDA warned about dosing errors with compounded semaglutide, including mix-ups between mg, mL and units (FDA alert). The same math applies to any vial.

Questions

How many mL is 12.5 mg of tirzepatide?

It depends on the vial: 2.5 mL at 5 mg/mL, 1.25 mL at 10 mg/mL, 0.833 mL at 15 mg/mL, 0.735 mL at 17 mg/mL, 0.625 mL at 20 mg/mL, 0.5 mL at 25 mg/mL, 0.417 mL at 30 mg/mL.

Is 12.5 mg of tirzepatide the same number of units on every vial?

No. The units depend on the strength printed on the vial: 250 units (more than a 1 mL syringe holds) at 5 mg/mL, 125 units (more than a 1 mL syringe holds) at 10 mg/mL, 83.3 units at 15 mg/mL, 73.5 units at 17 mg/mL, 62.5 units at 20 mg/mL, 50 units at 25 mg/mL, 41.7 units at 30 mg/mL.
