On a U-100 insulin syringe, 5 mg of tirzepatide is 100 units at 5 mg/mL, 50 units at 10 mg/mL, 33.3 units at 15 mg/mL, 29.4 units at 17 mg/mL, 25 units at 20 mg/mL, 20 units at 25 mg/mL, 16.7 units at 30 mg/mL.

At 10 mg/mL, the strength most often seen on compounded labels, it is 0.5 mL, which is 50 units. Use the calculator with the strength printed on your own vial.

Real vials can hold slightly more or less than the label says, and a little is lost in the needle.

Same dose, different units

At 10 mg/mL, 5 mg is 50 units. Drawing those same 50 units from a 20 mg/mL vial gives 10 mg, not 5 mg. Always check the mg/mL on the vial you are using.

In July 2024 the FDA warned about dosing errors with compounded semaglutide, including mix-ups between mg, mL and units (FDA alert). The same math applies to any vial.

Questions

How many mL is 5 mg of tirzepatide?

It depends on the vial: 1 mL at 5 mg/mL, 0.5 mL at 10 mg/mL, 0.333 mL at 15 mg/mL, 0.294 mL at 17 mg/mL, 0.25 mL at 20 mg/mL, 0.2 mL at 25 mg/mL, 0.167 mL at 30 mg/mL.

Is 5 mg of tirzepatide the same number of units on every vial?

No. The units depend on the strength printed on the vial: 100 units at 5 mg/mL, 50 units at 10 mg/mL, 33.3 units at 15 mg/mL, 29.4 units at 17 mg/mL, 25 units at 20 mg/mL, 20 units at 25 mg/mL, 16.7 units at 30 mg/mL.
